Diablo 4 Guide Update: Wrathful Hearts Farm

As you progress into higher tiers like Tier 3 and Tier 4 in Diablo 4 Season of the Malignant, you can still revisit the Wrathful Hearts Farm as long as you haven't completed the quest. The best part is that the hearts scale, meaning you have a chance to get various types of hearts from this farm. I've personally received Caged Heart of the Barber, Caged Heart of Revenge, and Caged Heart of Tal'Rasha from this farm. However, the drops are based on random number generation, so the specific heart you get will vary.


Earn of Malignance Boost

In the seasonal tab, you'll find the Earn of Malignance, where you can add ashes to unlock perks. Putting points into the Earn of Malignance will grant you a boost to the chance of powerful Malignant heart drops. After completing the initial campaign, you can add up to 4 points into the Earn of Malignance, providing you with up to a 15% chance to get boosted heart drops.


Level 4 and 5 Heart Drops

Testing the Wrathful Hearts Farm on Tier 4, I can confirm that you can get level 4 and level 5 hearts from this farm. However, it's worth noting that the availability of level 5 hearts on Tier 4 may vary. Some players have reported getting level 5 hearts without investing points into the Earn of Malignance, but I found that using it improved my chances of getting more powerful hearts and even the elusive Caged Heart of the Barber.


Consistency of the Named Heart

One thing that caused confusion is that you will always get a heart named "Varshan's Heart" every time it drops. The name of the heart remains the same, while the attributes on the heart are what change. So, don't be alarmed if you keep seeing Varshan's Heart dropping repeatedly; it's the attributes that can differ.
